Waiver And Release Of Liability
I acknowledge and understand that participating in strength, flexibility, and aerobic exercise, including the use of equipment such as Reformer Pilates machines, barre equipment, and suspension fitness tools, involves inherent risks. These risks include, but are not limited to, potential injury, especially for individuals with pre-existing conditions, illnesses, or medical disabilities.
By voluntarily participating in these activities and using the equipment and machinery provided, I accept and assume full responsibility for any risks of injury, illness, or even death that may result.
I further affirm that I am in good physical health and free from any condition, impairment, disease, or other medical issue that would prevent or limit my participation in these activities or use of the equipment. I acknowledge that I have been advised to consult with a physician before engaging in any exercise program or using exercise equipment. If I choose not to seek medical advice, I assume full responsibility for any consequences arising from my participation.
I understand it is my sole responsibility to inform Cat Fitness, LLC of any physical conditions or limitations that might affect my ability to participate in fitness activities. I also acknowledge that Cat Fitness, LLC and its staff do not diagnose or treat medical conditions or provide medical advice.
Assumption Of Risk And Release Of Liability
I expressly assume all risks associated with my participation in exercise programs, including Pilates, barre, and other fitness activities offered by Cat Fitness, LLC. I release, waive, and discharge Cat Fitness, LLC, its officers, employees, trainees, contractors, and shareholders from any and all claims, liabilities, or demands that may arise due to injuries or damages related to:
- My participation in fitness programs or use of any equipment or facilities.
- Slips, trips, or falls occurring in the building or surrounding premises, including sidewalks and parking areas.
I also understand and agree that Cat Fitness, LLC is not responsible for any loss, theft, or damage to personal property in or around the studio.
Acknowledgement And Agreement
By signing this waiver, I acknowledge that I have read, understood, and voluntarily agreed to the terms outlined above. I further confirm that I understand the risks involved in participating in fitness activities and using equipment provided by Cat Fitness, LLC.
